How to Submit the One Week Attendance Form

All instructors are required to submit the One Week Attendance form on My Union for the Fall and Spring semesters. This form is used to track attendance for federal financial aid purposes.

  1. Click the following link and log in with your Union account: https://my.unionky.edu/ICS/Academic/Portlets/Instructor_Portlets.jnz?portlet=Student_Forms&formname=One+Week+Attendance+Form

  2. Follow the instructions on the form, click Next page, and Submit.

  3. Repeat for each course you are teaching.
